4-H Enrollment for New Members

Use this sheet for Fashion Review, ATV/Tractor Contests, Horticulture Contest, Music Contest, Food Revue, Wildlife Contest, Shooting Sports, Theater Arts Contest, Judging Contests, Presentations, and Show & Tell.
Small & Large Animal (Green) Entry Sheet or (PDF format)
All large animal, including horse, and small animal exhibits. Fill out completely and legibly all that pertains to your exhibit. Livestock Showmanship classes must be entered on this Green Sheet.

Animal ID Sheets
Once you have completed a form, print one copy and bring it to the Nemaha County Extension Office.

This ID Sheet requires a clear photograph of the animal for the ID to be complete. Take photographs early.

Ear tags are available at the Extension Office for County Fair, Ak-Sar-Ben, and State Fair.
Tattoos must be legible.
Market Beef, Market Sheep, Market Swine, and Market Goats exhibiting at State Fair/Ak-Sar-Ben need DNA samples collected.
**If ID Sheet # does not match the animal tag or tattoo #, the animal will not be allowed to show.**
